You can download and
print an entry form by clicking on the button below.
Please complete all sections in full and enclose your
cheque for payment – incomplete entries cannot be
processed and we will have to return them to you.
Please take extra care when completing your address as
this is where your race pack will be sent. A
separate entry form is required for each participant
(though one cheque to cover all participants is
accepted). All entry forms need to be signed by an
adult. Please also include a stamped addressed
envelope as stated at the bottom of the entry form.

The cost for all Junior
Races is only £1 per child in advance and for the
Monster Dash 5Km Fun Run its only £5 per person in
advance. Please make cheques payable to Monster

Protection
Your details will only
be held at Fylde Coast YMCA and not passed on to any
other organisation or third party when you send in your
entry by post or in person.
All participants and
spectators may be filmed, photographed or recorded for
broadcast, security, archive and health and safety
purposes.
Race
Instructions
Race packs will begin to
be sent out from July onwards. Processing time is
normally seven working days. You will receive in
the post full instructions, a
sponsorship form and your individual race number.
Your race number is acceptance of entry and all fees are
non-refundable. Monster Dash cannot be responsible
for applications lost in the post.
We would encourage you
to raise as much as you can for the charities we are
supporting. Please see our sponsorship section for
more details.
Closing Date
Races are likely to
reach capacity very quickly so please send in your entry
form(s) early to avoid disappointment.
Postal
registrations will close on Friday 6th August
2010. Entries received in the post after this date
will be returned to you. You can still obtain
these lower entry fees in person by visiting the St Annes YMCA on or before Saturday 14th August.
On
The Day
If
there are still spaces available, registrations can be
processed on the day at Stanley Park stadium.
Junior races have a £2.50 entry fee per child and the
Monster Dash 5Km Fun Run has an entry fee of £10 per
person. Marshals will be able to direct you to the
registration point. Staff will be there from 9.30am
to process your entry. We can also accept any
sponsorship money you have raised too (though you may
want to finish the race first!) Please note that
registration closes 15 minutes before each race starts
so give yourself plenty of time to get there.
